"*" indicates required fields Aussie Pet Mobile Pierce County Pet Release Form Form Version* Standard ADA Accessible At Aussie Pet Mobile Pierce County, we want your pet’s grooming experience to be as pleasant as possible. It is important to understand that some pets have different responses to grooming, regardless of the groomer’s skill and effort. We make every effort to make it a positive experience. Sometimes pre-existing conditions arise unforeseen to our groomers, such as skin allergies, shampoo allergies, moles, clipper sensitivity, matting and tangles, fleas, fear, behavioral issues, bone or joint sensitivity, heart condition, seizures, etc. It is essential that you, as the pet owner, inform us of anything that you are aware of prior to grooming. Scheduling: Scheduling is a critical part of our business; we strive to reduce our drive times so that we can provide the best service to the most pets on any given day. On occasion, we will contact clients to reschedule to a different day or time to reduce travel time. The nature of our work – dealing with pets and traffic (and occasional mid-day cancellations) – means that all appointment times are approximate. If a groomer is significantly behind or ahead of schedule, we will attempt to notify you with the best information we have at the time. When possible, please schedule on a day that you or a family member will be home and not overly pressed for time. Fleas & Ticks: To prevent transfer of fleas from one appointment to the next, it is our policy to reschedule appointments immediately following a pet with fleas, to fumigate the van and tools, and for the groomer to shower and change uniform. If you suspect your pet has fleas or ticks, please inform us prior to scheduling the service so that we can schedule you at the end of the grooming day. Please note that parasites are a health hazard to your pet as well as to humans. If fleas or ticks are found during the grooming process, your pet will be given a treatment for such and you will be charged an additional fee. If we need to cancel other clients’ appointments, we will also charge you a van fumigation and clean up fee. If your appointment is the last of our grooming day, we will waive the van fumigation and clean up fee. Matted coats and Excessive undercoats: Unmaintained coats cause a variety of health problems for your pet. Fleas, ticks and other parasites may be lurking in the coat causing further skin infections. The skin underneath can be raw and inflamed. Severely matted coats will require a shave down. We are happy to show you how to care for your pet’s coat in the future; however, we cannot brush out a coat that is already severely matted due to the stress and the pain it will cause your pet. Your pet may be nicked or cut because we have to work so closely to the skin to remove the coat. We are very careful, but it is possible an injury may occur. Additional matting charges will apply, which your groomer will explain before performing services. Current Vaccinations/Veterinarian Information: Puppies and kittens must have completed all their shots before the first groom. By signing this contract, owners verify their pets are current on Rabies, Distemper and Parvovirus vaccinations. Aggressive or Dangerous Pets: Owners MUST inform us if your pet(s) bites, has bitten, or is aggressive to people, other pets, or during specific grooming procedures. Muzzles may be used if necessary. Muzzling will not harm your pet and it protects both the pet and the groomer. We reserve the right to refuse/stop services for such pet(s) at any time before or during the grooming process. Special handling fees may apply. Health or Medical Problems & Senior Pets: Grooming procedures can sometimes be stressful and can expose hidden medical problems or aggravate a current one during or after the groom. Because senior pets and pets with health problems have a greater chance of injury, the groomer’s first objective for these pets is cleanliness and comfort. Elder care fees may apply. Note that we do not accept cats over 15 years of age unless we have a prior history with them. We are unable to safely groom pets with severe hip dysplasia, severe seizure disorders, or severe heart conditions. We suggest you have them groomed at a veterinary office. We do not express anal glands. Accidents: There is always the possibility an accident could occur. Grooming equipment is sharp; even though we use caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. Every effort will be made to ensure your pet is groomed as safely as possible. In the event that an injury occurs to your pet during the groom, the groomer will take care of the injury according to our safety and accident policy. And in the case of a serious injury, he/she will inform you immediately. Appointment Cancellation: We understand unexpected events occur out of our control; therefore, no action is taken the first time a client cancels within 24 hours of the appointment. However, the second time this happens we assess a $50 late cancellation fee. If they “no call, no show” for any appointment, a $50 fee will be assessed. Tips are always appreciated but never expected. If a groomer does a particularly good job and/or goes beyond your expectations, then a tip of 10-20% is typical. 100% of tips go to your groomer. Payment for grooming services is due at the time of service. We accept cash, check, credit, and debit cards. Note that we use a third-party service (currently Square) for processing credit and debit cards; if you authorize storing your card information, it will be stored by the service – not by Aussie Pet Mobile. There is a $25 fee for returned checks. Hold Harmless Agreement: I hereby release Aussie Pet Mobile Pierce County from any responsibility of accidents, parasites, medical problems or other problems that may be uncovered while the grooming service is being performed. I hold harmless Aussie Pet Mobile Pierce County from any damage, loss, or claim arising from any condition of the undersigned pet, either known or unknown. It is also further understood and agreed the terms of this agreement can change at any time, without notice, and will override any and all prior signed contracts or releases.
